Mixing report and observation
Keep what the client reported distinct from what the counsellor directly observed whenever the format calls for that distinction.
Intuiti's live schema uses Progress for P. Some external GIRP guides use Plan instead; this page and download describe the product's actual four fields.
Live product field map · Progress, not Plan · Counsellor-owned clinical judgment
In Intuiti's live format, GIRP stands for Goal, Intervention, Response and Progress. The first three fields record the counsellor-set goal, the intervention used and the client's observable response. The final field is the counsellor's assessment of progress toward the goal. Other publishers use P for Plan; that editorial variant is not the field currently rendered by Intuiti.
Choose this product format when the counsellor has set a concrete goal and wants the note to connect an intervention and response with a counsellor-reviewed statement of progress. If no progress can be established from the session source, record that uncertainty rather than manufacture movement. BIRP is the closer fit when the immediate behavior–intervention–response trace matters more than a goal.
The goal this session (or this part of the trajectory) is working toward — stated briefly and concretely.
Which intervention you apply to work toward that goal.
How the client responds to the intervention, and what that says about progress toward the goal.
The counsellor's source-grounded assessment of progress toward the stated goal, including no observable progress or an unresolved result. This is not a generated treatment plan.
A fictional example — for illustration only, not based on a real client.
Draft — for illustration onlyG — Goal
Goal: learning to set boundaries with colleagues at work.
I — Intervention
Practiced a role-play in which client K. declines a colleague's request.
R — Response
Client says the role-play feels tense, completes one refusal and names which wording felt usable.
P — Progress
Counsellor records limited in-session progress: the client completed one practice attempt; use outside the session has not yet been observed.
Use the section headings below as an on-page starting point. Keep only source-grounded facts and write every clinical Assessment, Plan or Progress judgment yourself.

An AI draft is not the counsellor’s Assessment, Plan or Progress judgment. Add only the interpretation and next steps you independently stand behind.
Check names, timing, quotations, interventions and agreed plans against the eligible session source before separately saving the record.
GIRP is not used consistently across publishers. Some guides expand P as Plan. Intuiti's current schema and editor label it Progress. This guide chooses product truth and does not imply a Plan field the product does not have.

When you request it, Intuiti prepares a GIRP draft from the eligible source, for you to review. Intuiti does not add a separate Plan field.
Requested AI draft — you review and edit before a separate save-to-record action
Do not use an AI draft to invent or decide diagnosis, safety or risk status, referrals, assignments, treatment changes, Assessment, Plan or Progress. A draft may organize only what the eligible source actually records. The counsellor checks that source, supplies the clinical judgment, and decides what enters the record.
External references explain documentation conventions. Local law, professional standards and practice policy take priority.
Intuiti's GIRP starts from a concrete treatment goal and ends with Progress. BIRP foregrounds Behavior and ends with Plan.
No — the goal can differ per session or evolve across several sessions, as long as it's concrete enough to read progress against.
You can request a GIRP draft, then review it before a separate save-to-record action.
No. Intuiti's GIRP has no Plan field, and the draft is never a treatment plan.
